Service Description
Virtual appointment for existing patients to renew certification.
Cancellation Policy
To cancel or reschedule, please contact us twenty four (24) hours in advance. Cancellations made in advance or due to failure to qualify may be refunded. We also provide for one appointment rescheduling free of charge. Subsequent reschedule requests may be subject to a $25 fee. We do not offer refunds after your appointment if you are denied by the state for any reason.
