How to Finalize Your Application for Your Medical Marijuana Card

This guide is for patients who have already completed their face-to-face new patient appointment with a KYMMC physician.

If you haven’t had your appointment yet

Step 1

Register or Log In

Go to ky-chfs-public.nls.egov.com/login
Click “Register” (top right corner) to create an account, or log in if you already have one.

Step 4

Upload Your Notarized Form

Scan or take a picture of your Patient Signature Application Page.
Upload the form to the portal on the fifth tab.

Step 2

Start a New Application

From the dashboard, click “+ Create New Application” (left side of the screen - green button).
Choose “New Patient Registration” and follow the prompts.

Step 5

Pay Your Background Check Fee

Go to the seventh tab and pay the $25 fee for the state-mandated background check.

Step 3

Accept Your Certification

On the Practitioner Written Certification Information tab, click “View Available Certifications” and select our certification.
